Thank you for referring your patient to us. We will provide them with the best care possible.
Due to the high volume of referrals we receive, we have been unable to respond in a timely manner to requests for new patient appointments. It is now possible for patients to schedule the best time for us to contact them to discuss their new patient appointments. Below are the options that you can use to send us a new patient referral.

To avoid any delays in scheduling the patient referral, please fax over the request and have the patient “Schedule a Call” for one of our Intake Coordinators to contact the patient.

SCHEDULE A CALL
Select a date and time that’s best for our Intake Coordinator to contact the patient to schedule their appointment. They will be receiving a call from either our Wendell or Wilson office from Sonia or Khalil.
