New Patient Appointment Information
Please review the following policies before scheduling an appointment:
All new patient appointments will be scheduled with our provider: Nicole Brooker, PMHNP.
Important Policies:
- Benzodiazepine Prescriptions: Our providers DO NOT prescribe any benzodiazepines (benzos). Your current provider may continue prescribing your benzodiazepines. If you are willing to taper off your benzodiazepine, we can help you with this.
- Urine Drug Screen: If the patient is 13 years and older, they will be required to provide a urine sample during the first in-person visit.
- Credit/Debit Card on File: We require a credit/debit card on file for no-show fees or co-pays. A $75 fee applies if you cancel without 24-hour notice. Medicaid patients will be not be charged a fee, but will be discharged after the second no-show.
- Emotional Support Animal Letters: We DO NOT provide emotional support animal letters.
- Disability Paperwork: We rarely complete disability paperwork and only in extreme cases at the provider’s discretion. Do not book if this is your main reason for seeking care.
Steps to Schedule Your Appointment:
- Click the button below.
- Select an appointment date and time.
- We will receive your appointment request.
- If the date and time work, we will contact you to confirm your appointment.
- You will receive a text and email to create your portal and fill out new patient paperwork.
- You must fill out new patient paperwork as soon as you receive the text and/or email. If it is not filled out 24 hours before your appointment, your appointment will be canceled.
